A completed essay as described in Section 7 of the application.
Three original letters of recommendation. Two recommendations MUST come from current teachers, counselors or school administrators on official letterhead or on school letterhead with the signature and title. One recommendation MUST come from a non-family community member (such as employer, minister, coach, etc.). This recommendation does not have to be on official letterhead. All reference letters must be received sealed, with the signature of the person providing the reference across the sealed flap of the envelope. All reference letters MUST be submitted by the application deadline or the application will be considered incomplete. Be sure to let the person writing your letter know the deadline is March 12, 2010.
Official scholastic transcript(s) (high school or college), including the fall semester or quarter of 2009 AND current enrollment verification (both documents MUST be submitted).
Official copy of SAT Scores MUST be submitted. Failure to submit these documents will result in an incomplete application.
A copy of the first page of the 2009 Federal Income Tax Return for any person(s) (including yourself) contributing to your education.
Copy of Proof of United States Citizenship (birth certificate, if born in the U.S., passport or U.S. citizenship papers, if applicable) or legal non-citizen resident status. Originals will be required at interview, if scheduled.
A recent photograph.
Scholarships will be awarded only to applicants pursuing a health-care profession including, but not limited to::
St. David's Foundation will accept applications from students pursuing a degree in psychology or social work ONLY if the student plans to work in a medical environment (i.e., hospital, clinic, etc.).
Consideration will not be given to a student who wishes to pursue a career in a health-related field not pertaining to humans.
Application Procedure
It is your responsibility to see that the following items are received in the Foundation office: your completed application, essay, photograph, transcripts, tax form(s), letter(s) of recommendation and copy of proof of U.S. citizenship or legal residency are due no later than March 12, 2010 at 4:00 PM. Applicants not meeting these deadlines will not be considered. THERE ARE NO EXCEPTIONS.

Interviews In person interviews will be held the week of May 10, 2010.
Awards If you are awarded a scholarship, you will be notified by May 17, 2010. Scholarship funds for tuition are sent to the Financial Aid Office of your school. You must request that the Registrar or Financial Aid Office send us verification of enrollment as a full-time student in a health professional program for the fall semester or quarter. At that point, scholarship funds will be sent to the Financial Aid Office of your school. A scholarship recipient may be awarded a scholarship for up to four (4) years, if qualified. Post-graduate scholarships may be awarded pending the student maintain the minimun requirements.

Your application will not be considered unless all documents are in the Foundation office no later than March 12, 2010 at 4:00 PM
The week of May 10 Interviews will be held during this time. Advanced notification of the interview schedule will be sent to the applicant.
